Job Description

At the heart of integration is the vision to produce better outcomes for people through services that are planned locally and delivered seamlessly from the perspective of the patient, service user or carer.

We are now looking to appoint an Integrated Operations Manager (Adult and Hospital Discharge Services) to ensure that the integration agenda continues to be achieved at a local level and will assist the Head of Community Health and Care, in delivery of the HSCP objectives within the Strategic Plan.

To build on the current progress and provide Leadership and vision with responsibility for the delivery of effective services within an integrated model of care provision, you will be expected to fully engage with all of the key stakeholders and foster a partnership approach to delivering care whist always ensuring that the service user is at the centre of decision making.

You will lead on aspects of the Clinical and Care Governance agenda in respect of the ongoing
development of Adult and Hospital Discharge services. The successful candidate will be expected to have the necessary skills, experience and knowledge of working practices of more than one staff group including NHS and Local Authority Staff Groups. In particular there is
an expectation of the successful candidate having excellent communication skills in managing people.

In addition the candidate should demonstrate evidence and knowledge of the strategic service agenda in relation to the integrated model of service delivery as well as managing change.
