Role Profile


To assist individual service users to remain in their homes, or facilitate their discharge from hospital, by assisting them to achieve maximum independence, undertaking the full range of tasks as detailed below on either a planned basis or in an emergency situation.


Corporate Responsibilities

  • To undertake individual role in line with the Council’s Strategic Priorities and in compliance with the Council’s corporate policies and procedures.
  • To undertake any other reasonably required duties as instructed by Management or someone acting on their behalf, in addition to the role specific tasks & responsibilities detailed below.

Role Specific Responsibilities

  • Providing care and support to service users within the community, as detailed within their care plan and as instructed by the appropriate Line Manager, including:
  • Assistance with personal care tasks, including bed making, bathing, bed bathing, assistance to toilet, assistance with dressing.
  • Assisting service users with rehabilitation techniques, as directed.
  • Financial assistance, including banking and payment of accounts and collection of pensions (where appropriate arrangements have been agreed with the Line Manager) and assistance with budgeting, money management and explaining financial transactions.
  • Assistance with various practical household tasks to help the service user maintain standards of hygiene in the home.
  • Understanding the dietary requirements of older people, assisting with menu planning and management of food stores, managing food hygiene, along with the preparation and cooking of meals and providing supervision during meal times.
  • Shopping and collection of prescriptions
  • Assistance with communication as detailed in care plans, writing letters, reading correspondence, newspapers, shopping lists etc.
  • Assistance with domestic pet care as per care at home procedures.
  • Monitoring safety issues within the service user’s home and advising / assisting with health and safety issues.
  • Responding to emergency situations, using initiative to identify and take appropriate action, in line with Service training and procedures.
    • Providing assistance with extended personal care tasks where required, namely:
  • Support with medication and oxygen care
  • Continence, Urinary and Stoma care, as required
  • Specific feeding care
  • Mobility and support with rehabilitation
  • Assistance with personal hygiene
    • Monitoring, recording visits and keeping others up to date
  • Monitoring the care and wellbeing of service users, recording visits and incidents in the personal record of care or other appropriate documentation, updating personal information in accordance with the service user’s wishes.
  • Keeping the appropriate Line Manager informed about the service user’s progress and advising of any changes in the service user’s circumstances or needs.
  • Liaising with other individuals or services on the service user’s behalf and in accordance with their wishes.
    • Working effectively with others:
  • Providing respite for carers.
  • Working closely with health colleagues to deliver personal care tasks.
  • Liaising with other agencies as required.
  • For Community Alarm posts only:
  • Respond to calls from the Community Alarm Station employees by visiting service users as directed and taking appropriate action.
  • Report accurately to appropriate personnel the need to call emergency services such as GP, Ambulance, Police, etc.
  • Contact neighbours, next of kin and named contacts as appropriate.
  • Install and check Community Alarms / Telecare (SMART) equipment as directed
  • Assist and advise service user of operation of equipment.






Education and Qualifications


  • SVQ Level 2 Social Care, or equivalent
