Location: Hairmyres Hospital, Eaglesham Road, East Kilbride, G75 8RG and Wishaw General Hospital, 50 Netherton Street, Wishaw, ML2 0DP

Fixed Term: 1 year

Vacancies have arisen for Community Support Co-ordinators within one of the largest services delivered by the council. Over 1,000 Home Carers provide essential support to over 2,300 service users to enable them to stay at home.

This post offers excellent opportunities at a time of considerable development within the service. As a Community Support Co-ordinator based within the hospital you will play a key role in the delivery of community services to support hospital discharge. You will be responsible for the coordination of packages of support, linking with community teams and home care staff and the delivery of care services, in line with Supporting Your Independence across South Lanarkshire Council. You will be link with community teams and should be able to demonstrate excellent organisational and communication skills. You will also have responsibility for managing and organising the care for service users.

You will work as part of a multi disciplinary team assessing service users’ needs in respect of care referrals and monitoring and reviewing care within agreed timescales. You should have excellent IT skills and be dynamic and creative, with the ability to resolve problems as they arise.

Successful applicants will be required to be flexible in approach and to travel across various locations within South Lanarkshire.

It is essential that applicants either possess, or are willing to work towards a recognised qualification in social care management and gain SSSC registration within 6 months of commencing.

This is a temporary position for a period of up to 1 year.  Prior to making your application, please discuss your potential application with your current Line Manager.  In the event that you are being considered for this post, your release to undertake this internal fixed term opportunity would need to be agreed in advance and managed by the service/s or resource/s.
